Consent Of Data Subject to The Processing of Personal Data

The consent of data subject to the processing of personal data granted under Art. 6 and 7 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27th April 2016, on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (hereinafter as "Regulation" or "GDPR") and according to Sec. 13 of Act No. 18/2018 Coll. on Personal Data Protection (hereinafter referred to as the “Act”, “Act No. 18/2018 Coll.” or the “Personal Data Protection Act”)

DATA SUBJECT IDENTIFICATION DATA

A data subject is any individual (job applicant) who confirms his / her interest in being registered in the controller’s database of job applicants.

I, as a data subject, hereby voluntarily give my consent to pretlak s. r. o. as the controller to process my personal data within the scope of a CV, cover letter, photo, and other attached documents.

PURPOSE OF PERSONAL DATA PROCESSING

The purpose for processing the personal data of the data subject is to provide assistance and support in finding a new job and to manage the related (i) registration of a CV, cover letter, photo or other attached documents into an internal database of job applicants maintained by the controller, as well as (ii) to make the personal data of the data subject available to the controller's business partners, who express their interest in a candidate, whose profile matches the expertise and work experience of the data subject.

PERIOD FOR PROCESSING THE PERSONAL DATA

I give my consent to process my personal data within the scope defined above, to the controller for a period of eighteen months from the date of sending the documents to the controller / uploading them to the website. After fulfilling the purpose of processing personal data or after the expiration of the period defined above, the controller shall ensure without undue delay the disposal of the personal data, i.e. deleting and removing it from all data storages, or shredding / disposing of paper documents. The controller shall ensure an adequate level of personal data protection, shall process personal data solely in accordance with accepted principles of morality and shall act in a manner that does not infringe the Regulation, the Personal Data Protection Act or other generally binding legislation.

TRANSFER OF PERSONAL DATA TO THIRD COUNTRIES OR INTERNATIONAL BODIES

The controller does not perform such transfer.

CATEGORIES OF RECIPIENTS

The personal data of a data subject may be made available to controller's business partners, who express their interest in a candidate, whose profile matches the expertise and work experience of the data subject. The personal data may also be made available to a web hosting provider for storage and backup of data, as well as to an IT service/maintenance provider (for viewing only).

INSTRUCTIONS ON THE DATA SUBJECT’S RIGHTS

This voluntary consent to the processing of personal data may be withdrawn by the data subject at any time and free of charge, in writing at the address of the controller's registered office or by sending an email to [email protected]. When processing personal data, the controller does not use any form of automated individual decision-making, including profiling pursuant to Sec. 28 Par. 1 and 4 of the Act. The data subject has the right to request the controller to provide access to his / her personal data and has the right to correct or delete it, or restrict its processing, or the right to object to the processing, as well as the right to data portability. Data subject has the right to defend his / her rights through the person in charge (if designated by the controller) or by submitting a complaint to the supervisory authority, i.e. Office for Personal Data Protection pursuant to Sec. 100 of Act No. 18/2018 Coll. on the protection of personal data.
